Who can apply for Adult Day Care?
Our day care facility is open to individuals who wish to use both private funding or Medicaid. In both instances, eligibility requires that the individual is at least 18 years old with a medical condition requiring treatment or rehabilitative services prescribed by a physician; or has an existing physical or mental impairment which handicaps activities of daily living; or has a reasonable expectation that preventative services will improve or maintain their present level of functioning.
What Is Community Long Term Care’s role in administering Adult Health Day Care?
Community Long Term (CLTC) operates home and community-based waiver programs for persons eligible for nursing home care but who prefer to receive their services in the community. Through a process of case management and an individualized service package, waiver clients can successfully remain at home at a cost to Medicaid that is substantially less than the cost of institutional care.
The Community Choices waiver serves the frail elderly and persons with physical disabilities who meet the nursing facility level of care criteria. The direct administration comes through thirteen (13) SCDHHS offices around the State, each of which covers designated counties of South Carolina. Case managers working in these 13 offices are responsible for ensuring that participants are aware of their service options and can make informed choices as to which form of service delivery they prefer. The Community Choices waiver provides participant directed options for supervision of services. The Community Choices waiver offers a continuum of service options capable of meeting the needs of all waiver participants, both those who choose agency directed services and those who choose self-directed services.
Services included through CLTC’s Community Choices waiver includes but are not limited to: adult health day care, home-delivered meals, attendant care, respite care, limited incontinence supplies, and non-emergency medical transportation services.

Operations
Where do we operate?
Our day care is located at 112 Carn Street, Walterboro, SC. We are authorized to provide adult health day care and transportation services statewide.
What are our hours of operation?
In order to accommodate the schedule of working families, Tucker’s Adult Health Day Care is open from 7:30am to 5:00pm Monday through Friday.
Is transportation available?
We readily provide transportation to and from out facility for all day care participants.
What is the staff to client ratio at our facility?
The South Carolina Department of Health and Environmental Control requires a minimum staff to client ratio of 1:8. Our ratio is typically 1:6.
